Please print, fill out, sign and return to us as soon as possible with a copy (or scan) of your photo ID.

In order to prepare your tax returns we require a copy (or scan)of your prior tax returns you filed, including federal and any state returns, personal, business, partnership or trust. This will allow us to set you up in our system and review your specific tax situation.

After we receive the signed completed New Client Packet and required information,   someone from our accounting team or payroll team (if applicable) will reach out to you in 3-5 business days.

We are excited you have decided to allow us to serve your tax and accounting needs. 

Sending Information to our Office:

You can send via:

  • US mail, UPS or FedEx that can be traced and confirmation of delivery sent to your attention
  • Fax information to 309-663-7277/888-315-7116
  • Email to info@hjerpecpa.com
  • Hjerpe & Tennison online portal (hjerpecpa.com) – Please see our home page to request access.  This is our preferred method of electronic delivery.  This is the most secure AND efficient way to send information electronically. 

We will not accept any documents sent via Google docs, please use our secure online portal.
